Contact and referrals

Discuss support or make a referral

Tell us what matters to you. Whether you’re looking for support for yourself or referring with permission, share your suburb in Victoria and the everyday support you’d like to discuss.

A simple first step.

You can contact us directly, or ask a family member, carer or support coordinator to help. If you’re referring someone else, please make sure you have their permission.

We welcome enquiries from Melbourne and across Victoria. Availability, worker suitability and travel arrangements are discussed before support is agreed.

You’re welcome to tell us how you prefer to communicate or ask for someone you trust to join the conversation.

Helpful information to share

  • Your name and preferred contact details
  • Your suburb or general location
  • The support or goals you’d like to discuss
  • Whether you’re enquiring for yourself or making a referral

Please leave NDIS numbers, medical records and other sensitive documents out of the first enquiry. We can discuss a suitable way to share relevant information later.

Your first conversation

You do not need to have every answer ready. It can help to think about:

Before deciding, ask what tasks, schedule, contact arrangements, costs, privacy and feedback process would apply. Availability and worker suitability need to be confirmed.
